You may be interested in the following investigative report on
the explosion of the Ariane 5 launcher in June this year:

The explosion, which was caused by a software error, constituted one of
the most expensive fireworks ever, given that the development costs of
the Ariane 5 thus far involve about 6 billion dollars.
